Hold a baccalaureate degree in occupational therapy from a program accredited by the American Occupational Therapy Association (AOTA);.

Satisfactorily completed at least twenty-four (24) weeks of supervised field work experience at a recognized educational institution or a training program approved by the educational institution where the academic requirements were met; and

Successfully completed the National Board for Certification in Occupational Therapy (NBCOT) certification examination.

Official transcript of occupational therapy education verifying degree awarded and date of graduation.
